Recently, genome-wide association studies revealed a close link between specific single nucleotide polymorphysms of the melatonin MT2 receptor locus and a prognostic risk of type 2 diabetes. In fact, these studies present evidence that a particular SNP significantly increases the risk of type 2 diabetes in the European cohorts examined. The coding sequence of MTNR1B is interrupted by a single intron and the SNP rs10830963 is localised within the non-coding intron sequence, although it does not interfere with consensus sequences for transcription factors or with splicing. However, this SNP is correlated with higher fasting glucose levels and a high incidence of this allele is also correlated with pathologically altered insulin secretion responses.

Medication For Sleep Woes
While there really arent any sleep aids out there designed specifically for PWDs , many often talk about Melatonin as an option to help with sleep issues. Melatonin is a naturally occurring hormone in your system that helps to control your sleep and wake cycles. Unlike insulin, Melatonin is a hormone that is synthetically made and can be ingested, so a natural supplement in pill form is available over-the-counter .
According to the American Diabetes Association, there is some concern that taking Melatonin could increase insulin resistance. But that hasnt stopped many from recommending it, trying it out, and building up a tolerance.
Fellow T1 Kelley Champ Crumpler in Texas, whos a diabetes nurse educator by trade, usually recommends Melatonin to treat sleeping problems.

Melatonin, a hormone that is normally produced naturally by the body, helps us to regulate our circadian rhythmour sleep/wake cycle. However, melatonin is often taken as a supplement for people who have difficulty sleeping for various reasons. Its one of the first things many therapists suggest when someone complains of insomnia.
There is a lack of compelling evidence that melatonin supplements can help with insomnia. Meta-analyses show that people taking melatonin fall asleep about four minutes faster, on average, than people taking a placebo drug, and may sleep about 10-15 minutes longer. But at the very least, it has been considered safeso even if it doesnt work, whats the harm?
Now, a new article reveals that melatonin may have harmful effects after allpotentially leading to type 2 diabetes.
The lead authors were Marta Garaulet and Jingyi Qian from the Medical Chronobiology Program at Brigham and Womens Hospital, Boston. The paper, published in Trends in Endocrinology and Metabolism, also included researchers from Harvard, MGH, and the Broad Institute , a medical research organization that focuses on genomics and large datasets.
People with polymorphisms in the MTNR1B gene may also have higher melatonin concentrations for a longer period of timewhich means that having breakfast in the morning may coincide with still-high melatonin levels.

When considering all the side effects and possible melatonin interactions with certain medication, the following groups should avoid taking melatonin

- Pregnant or breastfeeding women,

Researchers Conducted A Trial To Understand The Connection Between Disturbed Blood Sugar Level Control And The Timing Of Meals Relative To Sleep
Blood sugar level control, which is impaired in individuals with diabetes, is affected by various factors, including the timing of meals relative to sleep and melatonin levels, a hormone primarily released at night that helps control sleep-wake cycles.
Senior author, Richa Saxena, PhD, a principal investigator at the Center for Genomic Medicine at MGH, addressed the study: We decided to test if late eating that usually occurs with elevated melatonin levels results in disturbed blood sugar control.

Melatonin The Sleep Hormone And The Pancreas
Melatonin is produced by the pineal gland in the brain in response to darkness. Levels are typically very low during the day and reach their peak at night. Like other hormones, melatonin works by binding to compatible receptors kind of like a lock and key. These receptors are found abundantly in the eyes and the brain, and when melatonin binds to them, they signal that its dark outside. For humans, this darkness signal indicates that it is the period when we rest, so this timing signal contributes to and is a part of a cascade of other responses that help initiate and maintain sleep.

The consumption of melatonin has increased drastically in Denmark. The sale of melatonin increased from 2 million daily doses every quarter in 2015 to 6 million daily doses in the first quarter of 2021.
In Denmark, people increasingly take melatonin to improve their sleep, but does it have other effects on the body?
New research in Denmark shows that taking melatonin pills can also affect some gut hormones that are essential for maintaining stable blood glucose levels .
According to a researcher behind the study, the results indicate that we should think twice before prescribing melatonin for people with sleep disorders.
For many years, people have thought that melatonin is a natural sleep hormone that is not associated with significant adverse effects. We are investigating this through several studies, and so far they indicate that melatonin is not completely free of negative effects in glucose metabolism, explains Esben Lauritzen, PhD student and doctor at Steno Diabetes Center Aarhus and Steno Medical Research Laboratory, Aarhus University Hospital.

Melatonin and CNS-related complications of diabetes
Acute and chronic vascular and metabolic perturbations impair the structural and functional brain integrity in diabetic patients. In other words, DM contributes to various structural and functional disorders related to peripheral and central nervous systems .
In a study, Tuzuc and colleagues examined and compared the vitamin E and melatonin impacts on cognitive function of diabetic animals. Memory and learning behaviors were studies utilizing a spatial version of the Morris water maze test. Glutathione and lipid peroxidation levels were measured in frontal cortex and hippocampus. The diabetic animals developed considerable impairments in memory and learning behaviors in comparison with control rats. Moreover, the concentration of glutathione is reduced and the level of lipid peroxidation is enhanced in diabetic animals. Vitamin E and melatonin remarkably improved memory and learning performance. In addition, glutathione levels and lipid peroxidation were reversed by these nutraceuticals toward the control values. It can be concluded that, in diabetic patients, oxidative stress contributes to memory and learning deficits, suggesting that antioxidants including vitamin E and melatonin are able to ameliorate cognitive impairments in diabetes .

Adverse Effects Of Melatonin Intake: Overview
Consumption of food supplements that contain melatonin or mainly contain melatonin has shown adverse effects in numerous studies. The most frequently reported side effects include headaches, dizziness, drowsiness, tremors, migraine, nausea, vomiting, abdominal pain, nightmares, vivid dreams, and irritability. There were even cases of supplement abuse for suicide attempts through melatonin overdose.
The reason for such adverse effects was traced to an incorrect presentation of the amount of melatonin a bottle of supplements truly contains. Investigation shows that melatonin supplement bottles can have 4 times more melatonin than indicated. This can affect ones health significantly, because of the higher the dose, the serious and more frequent side effects.
However, this is not the only reason side effects occur. Sometimes, doctors do NOT prescribe a suitable dosage, or the patients decide to dose themselves as they want. Some people even tend to drink alcohol with melatonin, hoping to emphasize the sedative effects. But, they just cause even more serious side effects.

Melatonin Benefits When Fasting
1. Could help treat GERD
Gastroesophageal reflux disease, in short, GERD, is a condition that causes heartburn, nausea, and belching. It is caused by a backflow of stomach acid into the esophagus. Studies show melatonin can block the secretion of stomach acid and decreases the production of nitric oxide, which is responsible for relaxing the esophagus and allowing the stomach acid to enter.
GERD and fasting can be a very uncomfortable combination. While some people find that their symptoms get better when they fast, others report the opposite, so melatonin could be beneficial.
2. Could improve blood sugar management
Melatonin does not directly affect blood sugar or insulin levels. However, it is known that poor sleep comes with increased cravings, especially for sugar and salty foods. What Should I Do If This Happens
People and their partners or roommates should learn to recognize the signs of nighttime hypoglycemia. Be prepared! Ask your doctor for an emergency glucagon kit. This kit contains a fast-acting medication that can be injected if the person cant be woken up. Store the kit in a bedside drawer for easy access.
DO NOT PUT ANYTHING INTO A PERSON’S MOUTH IF THEY ARE ASLEEP OR CANNOT SIT UP.
If the person cannot be woken: If there is no emergency glucagon kit, call 911. If the person has a glucagon kit, the partner or roommate should follow the instructions to fill the syringe and inject the medicine. Once fully awake, he/she should eat a meal and check their blood glucose every few hours using a home test kit. After giving the injection, the persons doctor should be notified.
If the person can be woken up and sit without support: They should be given a fast-acting glucose source. Good options include hard candy, fruit juice or glucose paste or tablets, which can be purchased at most pharmacies. Once the person is fully awake, they should eat a meal and check their blood glucose every few hours using a home test kit.

Practice Good Sleep Hygiene
Most adults need at least seven to nine hours of sleep a night, according to the NSF. Many people dont allow themselves adequate time for sleep, Faiman says, but if you have type 2 diabetes, its critical that you do so. Dr. Strohl agrees, adding that its essential that everyone, but especially those with chronic conditions like diabetes, plan for adequate sleep just as you do other activities. Never think sleep isnt as important as watching TV or talking on the phone, he says. Another thing to keep in mind: Its important not to overdo it with naps. According to the NSF, naps should be kept relatively short around 20 minutes and limited to the early afternoon. Napping any later is likely to throw off your ability to get to sleep at night.
