Answer:Now, in a normal individual we measure blood sugar under different circumstances. What we call fasting blood sugar or blood glucose levels is usually done six to eight hours after the last meal. So it’s most commonly done before breakfast in the morning and the normal range there is 70 to 100 milligrams per deciliter.
Now when you eat a meal, blood sugar generally rises and in a normal individual it usually does not get above a 135 to 140 milligrams per deciliter. So there is a fairly narrow range of blood sugar throughout the entire day.
Now in our diabetic patients we see both low blood sugar levels that we call hypoglycemia, or elevated blood sugars, hyperglycemia. Now, if the blood sugar drops below about 60 or 65 milligrams per deciliter, people will generally get symptoms, which are some shakiness, feeling of hunger, maybe a little racing of the heart and they will usually be trenchant or if they eat something, it goes away right away. But if blood sugar drops below 50 and can get down as low as 40 or 30 or even 20, then there is a progressive loss of mental function and eventually unconsciousness and seizures. And of course that is very dangerous and a medical emergency.

Who Is Most At Risk For Developing Diabetes
The following categories of people are considered “high-risk” candidates for developing diabetes:
- Individuals who are overweight or obese
- Individuals who are 45 years of age or older
- Individuals with first-degree relatives with diabetes
- Individuals who are African-American, Alaska Native, American Indian, Asia American, Hispanic/Latino, Native Hawaiian, Pacific Islanders,
- Women who developed diabetes while they were pregnant or gave birth to large babies
- Individuals with high blood pressure
- Individuals with high-density lipoprotein below 25 mg/dl or triglyceride levels at or above 250 mg/dl
- Individuals who have impaired fasting glucose or impaired glucose tolerance
- Individuals who are physically inactive engaging in exercise less than three times a week
- Individuals who have polycystic ovary syndrome, also called PCOS
- Individuals who have acanthosis nigricans — dark, thick and velvety skin around your neck or armpits
In addition to testing the above individuals at high risk, the American Diabetes Association also recommends screening all individuals age 45 and older.
When To Seek Medical Care For High Blood Sugar
If hyperglycemia persists for at least two or three days, or if ketones appear in the urine, call a doctor.
Generally, people with diabetes should test their blood sugar levels at least four times a day: before meals and at bedtime . The urine should be checked for ketones any time the blood sugar level is over 250 mg/dL.
When blood sugar stays high despite following a diabetic diet and plan of care, call the nurse, diabetes health educator, or physician for adjustments in the diet.
If blood sugars are high because of illness, check for ketones and contact a health professional.
Seek immediate medical care for these conditions:
- Blood sugar levels that stay above 160 mg/dL for longer than a week
- Glucose readings higher than 300 mg/dL
- The presence of ketones in the urine

How To Keep Diabetes Under Control
In addition to modifying your diet, walk for 10 minutes after each meal to help improve insulin sensitivity and glucose uptake into your cells. Eat quality food and monitor your carbohydrates. Get a referral to a certified diabetes educator who will modify their recommendations based on your body, not what the guidelines say. If your blood sugar is still high with the recommended amount of carbs per meal, decrease the carbs further. Avoid low fat foods. Fat and protein will help your body absorb carbs more slowly and prevent spikes and troughs in your blood sugar.
Since diabetic diet is restrictive, you should change your view from focusing on the things you cant eat to focusing on all of the delicious things you can eat. All food groups are still on the table. You may have to modify recipes but there are so many awesome alternatives to almost any food available on the internet.
It is recommended to severely decrease your daily carbohydrates intake. However Low carb diets can still be rich and delicious.
Diabetics are recommended to avoid certain types of fruit, however, in moderation, all kinds of fruits can still be consumed. They key is to eat them in moderation. For example, half a banana is already one serving of carbohydrates. Apples and oranges should be no larger than a tennis ball. Be careful with dried fruits because they are concentrated.

It is important to work with your doctor to maintain healthy blood sugar levels. The following is a list of healthy blood sugar levels.
If an individual has type 1 diabetes the blood sugar should be the following:
- Before meals: From 90 to 130 mg/dL for adults

How Can One Tell If I Have Diabetes By Examining My Blood
Your body converts sugar, also called glucose, into energy so your body can function. The sugar comes from the foods you eat and is released from storage from your bodys own tissues.
Insulin is a hormone made by the pancreas. Its job is to move glucose from the bloodstream into the cells of tissues. After you eat, the level of glucose in the blood rises sharply. The pancreas responds by releasing enough insulin to handle the increased level of glucose moving the glucose out of the blood and into cells. This helps return the blood glucose level to its former, lower level.
If a person has diabetes, two situations may cause the blood sugar to increase:
- The pancreas does not make enough insulin

If left undiagnosed and untreated, the eyes, kidneys, nerves, heart, blood vessels and other organs can be damaged. Not Taking Enough Meal Time Insulin
This is the first thing to look into. Make sure that you correctly determine the number of carbohydrates in your last meal of the day and take the insulin to suit. Be sure to also look out for sources of hidden carbs as well. Our bodies react to food and insulin differently at different times of the day. Many individuals including myself have a different carb to insulin ratio at night. I normally have to take a bit more insulin at night especially if the meal is high in carbohydrates.
Try eating your last meal at least 2 hours before bed so that you can check the 2 hour post dinner blood sugar reading. This will indicate if you are indeed giving the correct amount of insulin. If you have a good reading before bed but its much higher in the morning, then there is definitely another issue.

Medications With A1c Of 63
Many doctors wont prescribe diabetes medication for someone with an A1c of 6.3. However, when other risk factors are present, such as high blood pressure or high cholesterol, your doctor might prescribe a first line drug to reduce your blood sugar.
The most common first line drug is Metformin, an oral drug that reduces glucose production in the liver, decreases the absorption of glucose in the stomach and improves your bodys insulin sensitivity.
Already on medication to manage your diabetes? If so, an A1c of 6.3 might be considered adequate, though getting below 5.6 is still recommended.
Talk to your doctor about whether an A1c of 6.3 is the optimal level for you and if medication, dosage or injection adjustments are necessary.
Veggies Or Fat Or Protein
I was taught to eat my salad first, then the meal but I read that you advised to break a longer fast with ¼ cup of Macadamia nuts, and an hour later a salad. Ivor Cummins talked about the veggies doing their thing in the upper intestine, and the fat and protein in the lower. My question is which should I eat first to receive the best benefit. I eat once a day while fasting at least 24 hours. When 24 hours comes past 8 pm, I wait until the next day around lunch, then move ahead one hour each day and repeat.
I think you should eat the salad first. There is some data to suggest that eating the veggies first reduces insulin.

What To Do If Your A1c Is 63
An A1c of 6.3 falls into the prediabetic range between 5.7 and 6.4. People with prediabetes are likely to get type 2 diabetes within 10 years unless they make serious changes to their lifestyle.
Type 2 diabetes is the most common form of diabetes. Type 2 means that your body still produces insulin but isnt using it properly. Many people can control their blood sugar levels with lifestyle changes while others may need insulin or other medications to manage it.
Keep an eye on your blood sugar by testing at home. Its easier than ever and there are a variety of affordable blood glucose monitors available.
A prediabetes A1c reading is a call to action. At a minimum youll need to make some lifestyle changes. You and your doctor can discuss whether medication is necessary.
A Note About Accuracy
In the USA, a blood glucose meter can be approved for sale so long as the results are consistently within 20% of the accurate value. That means that if your blood sugar is 180 mg/dL or 10 mmol/L, your meter may display a result of 216 mg/dL or 144 mg/dL . In practice, most meters are more accurate than that, but even an expensive hospital blood test using calibrated equipment has a 10% margin of error.
So dont panic if you check your blood sugar and its 140 mg/dL and a few minutes later, its 150 mg/dL . This may just be the normal discrepancy in the measurements. Look for trends over time to truly understand whats happening.
Similarly, CGM measurements also have a margin of error , and varies slightly depending on the specific system.

I dont generally recommend people to count calories. Fasting means not eating anything at all, so there is no counting macros. Dr. Mosleys 5:2 diet allows 500 calories of whatever you like. But thats his diet, not mine. I advocate intermittent fasting, with no calories.
